1. In consideration of being allowed to participate in the activities and programmes of LUV2SWIM Limited which are undertaken by LUV2SWIM Limited instructors and to use the facilities and equipment managed/owned by LUV2SWIM Limited and/or under the control of LUV2SWIM Limited, in addition to the payment of any fee or charge, I do hereby waive, release, and discharge LUV2SWIM Limited, its agents, employees and servants from any and all claims, losses, or liabilities for death, personal injury, partial or permanent disability, property damage, medical expenses, theft, or damage of any kind, including economic losses, which may in the future arise out of or related to my participation in the programmes/activities of LUV2SWIM or the use of its facilities for whatever reason.
2. I understand and am aware the swimming and related training including the use of equipment and facilities of LUV2SWIM Limited is a potentially hazardous activity. I also understand that exercise and fitness activities involve a risk of injury and even death, and I am voluntarily participating in these activities and using equipment and facilities with the knowledge of the dangers involved. I hereby agree to expressly assume and accept all and any risks of injury or death.
3. I hereby declare myself to be physically sound and suffering from no condition, impairment, disease or infirmity or other illness that would prevent my participation or use of equipment or facilities. I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in activity and utilisation of equipment and facilities in my activities.
4. I hereby declare myself to be physically sound and healthy prior to training. I also declare that I am not taking part in any LUV2SWIM Limited class under the influence of alcohol or other inebriating substances.
